NSAID Gastropathy: An Update on Prevention

چکیده
Correspondence to: Kam-Chuen Lai Abstract: Adverse reactions to non-steroidal anti-inflammatory drugs (NSAIDs) are common. Upper gastrointestinal complications including ulcer bleeding, perforation and obstruction are the most serious complications that can result in substantial morbidity and mortality. Many NSAID-using patients present with complications without any preceding symptoms. Risk factors for NSAID-related gastrointestinal bleeding include advanced age, past history of ulcers and bleeding and concomitant use of anticoagulants or corticosteroids. The role of Helicobacter pylori infection in NSAID-related peptic ulcer is unclear. In high-risk patients who need to continue NSAIDs, prophylactic treatment against ulcer development is required. Options include concomitant use of anti-ulcer drugs, use of selective COX-2 inhibitors and eradication of H. pylori. This review summarizes the current status of these regimens in the prevention of NSAID-related peptic ulcer complications.

Aging and the alimentary tract The aging stomach: implications for NSAID gastropathy
Over 30 million people worldwide use non-steroidal anti-inflammatory drugs (NSAIDs) daily, and nearly 50% of these NSAID users are elderly. 2 Numerous human studies have shown that the use of NSAIDs is associated with various gastroduodenal mucosal lesions, sometimes collectively referred to as NSAID gastropathy, and that NSAID gastropathy and its life-threatening complications occur primarily ...

OBJECTIVE To investigate the role of initial nonsteroidal antiinflammatory drug (NSAID) choice in the prevention of NSAID gastropathy, based on relative clinical and economic effects. METHODS To mimic clinical practice, a symptom-driven decision analytic model was constructed to compare 2 treatment strategies for long-term users of NSAIDs over a 1-year period: Strategy 1-generic NSAID used in...

متن کاملTargeting oxidative stress and inflammation in NSAIDs induced gastropathy: A plausible therapeutic approach
Deregulation in prostaglandin (PG) biosynthesis, oxidative stress, inflammation, and apoptosis play a major role in the pathogenesis of nonsteroidal anti-inflammatory drug (NSAID)-induced gastropathology. Although the underlying molecular mechanisms involved in this pathophysiological condition is not fully understood, considerable progress has been made in recent years. Also, substantial progr...

This prospective study aimed to determine the prevalence of Helicobacter pylori infection in relation to the occurrence and severity of NSAIDs induced gastropathy. A total of 111 patients were studied-66 were taking NSAIDs and 45 were control patients. All patients underwent endoscopy during which antral biopsy specimens were taken to determine H pylori status (Gram and Giemsa staining, urease ...
